f you live within our practice boundary and would like to register with us, please complete a registration form and health questionnaire below (or collect from our reception team). You have the right to express a preference of practitioner. However, you will be registering with the practice rather than an individual GP.
Catchment Area
If you have your medical card and NHS number, please show this to us. You will need to verify your identity and address, so please ensure you have photo ID (e.g. passport, driving licence) and one as proof of address (e.g. council tax utility or bank statement).
To complete your registration process, we request that all new patients joining the Practice see one of our nursing team for a general new patient health check.
Please let us know if you have any special needs or preferences for how we should communicate with you.
The Practice can provide you with information in easy-read format, Braille or translated leaflets. We can also access interpreter services to support your consultation.
Please just inform our Reception team of your needs and preferences.

Named GP
The NHS now requires all patients to be allocated a ‘named GP’ who is nominally responsible for your care at the practice. Regardless of who your named GP is, you can continue to access appointments with any GP or healthcare professional at the practice.
You will continue to be able to access appointments with any GP, Physician Associate, Nurse or Pharmacist at the practice, exactly as you do at present.
Your named GP will work in conjunction with the whole Practice Team to ensure the quality of your care.

If you have a very specific request to change your named GP, please submit your request in writing for the attention of the Practice Manager.
Temporary Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
