Onside Advocacy are delighted to work in partnership with The Corbett Medical Practice to deliver the new Neighbourhood Social Prescribing service.
Social Prescribing recognises that some of life’s problems can make you unwell and health conditions harder to deal with. Medical treatment may only be part of the solution. Social Prescribing offers options to link patients with local support sources to improve your health and wellbeing.
The Social Prescriber works with patients to identify the issues, difficulties and challenges they face and explore what is important to them. They support the patient to access local services and activities. This includes a wide range of options, e.g. advice and information on; housing, finance, benefits support, education and learning, healthy lifestyle, social activities and mentoring.
Lifestyle advice helps you feel healthier and happier by addressing lifestyle issues such as diet, smoking, alcohol and exercise. Our Advisor will work with you to identify an approach and support to help you achieve your goals.
Please speak to a member of the practice team to get a referral to the Social Prescriber.
